Illinois Primary and Immediate care

The Illinois Primary and Immediate care provide routine checkups as well as illness and injury care for patients of all ages. Our primary care physicians and pediatricians are committed to growing personal relationships with all the families we serve and providing you with the personal care you or your child deserve. We take great pride in providing exceptional care to our patients and offering treatment for everyday conditions from the common cold to chronic care.

When you require a specialist, our clinical staff will coordinate care with a medical specialist. We have see patients by appointment as well as take walk in at 815-455-1266 and are open Monday through Friday from 9:00am to 5:00pm and Saturday 9am to 12noon, except holidays. We accept most insurance plans, including Medicare and Medicaid with some exceptions.

The staff at Wesley Family Medicine is comprised of physicians who teach family medicine residents and medical students at the University of Kansas School of Medicine. The students and residents provide direct patient care along with our faculty primary care physicians and nurse practitioners. We provide comprehensive, high-quality healthcare for your family.

Our Family medicine specialists serve as primary care physicians, helping keep adults healthy and treating general health problems. They also treat chronic conditions and coordinate care with any specialist you might need.

Our primary care physicians serve as your gateway to medical care. We partner with our patients to help them prevent health complications, manage active disease processes and coordinate care with specialists. Our team of primary care physicians and nurse practitioners also provide annual physicals and treat everyday illnesses when they arise.

Our primary care physicians see patients by appointment also. Appointments are available between 9 a.m. and 4:15 pm.
To schedule an appointment please call (815) 455-1266. To cancel an appointment, please call the office at least 24 hours in advance.

If you fail to cancel an appointment three or more times without calling to cancel, you may no longer be allowed to use our services. If you arrive more than 10 minutes late for a scheduled appointment, you may be asked to reschedule.

For your privacy and the consideration of others, please turn off cell phones during your visit.

If you have an urgent care need after hours, please call (815) 455-1266. The operator will take your name and telephone number and contact the doctor on call.

What to bring to your appointment

  • Social Security number
  • Insurance card and the policyholder’s name and date of birth
  • Photo ID
  • All medications
  • Forms you need the doctor to complete
  • Consent for treatment if anyone other than the legal guardian brings a minor to an appointment
